Autologous PRP Wound Care Therapies
Autologous therapy is currently indicated for patients with chronic non-healing wounds that have been unresponsive to conventional wound therapy. Autologous therapy involves the use of aggressive wound debridement, excision and removal of devitalized tissue, and the repeated application of ThrombograftPRP to the wound site. The treatment regimen includes: Off Loading: Avoiding all mechanical stress on the injured extremity and minimizing trauma. Off loading can be achieved by the use of (1) total contact cast, (2) bed rest, (3) bivalve and other casts, (4) surgical shoes, half shoes, sandals, (5) felted foam dressings, and (6) other effective off loading techniques.
Debridement: Sharp debridement and removal of devitalized tissue at frequent intervals. Proper drainage techniques to prevent any pooling of purulent exudate.
Dressings & Providing Moist Environment: The use of ThrombograftPRP on chronic non-healing wounds not only provides a moist environment, but also protects the wound integrity while concurrently providing a medium for enhanced regranulation, re-epithelialization, cellular proliferation, migration, and extracellular matrix synthesis, the components necessary for accelerated wound healing. Dressing characteristics pertain to achieving absorption, hydration, conformability and moisture.
Management of Infection: Clinical diagnosis of infection with symptoms of (erythema, heat, warmth, tenderness, induration, pain, inflammation). ThrombograftPRP applications provides antimicrobial therapy. Other antimicrobial and antibiotic therapy must be considered.
Patient Education: Patients are informed of the importance of blood glucose normalization levels and diets containing proper nutrients to include adequate levels of protein, carbohydrate, vitamin C, B complex, A, and K, and minerals to include iron, copper and zinc. The importance of Off Loading is stressed to patients. Patients are encouraged to have a healthy attitude and maintain good psychosocial health which greatly promotes the attitude of the healing patient. The patient is encouraged to follow physicians orders, and are instructed in wound assessment skills so they may detect early signs of complications.
Prevention: Recurrence of foot wounds are common. Patients are taught changes in behavior that caused the original breakdown. Patients are taught foot care, nail care, proper footwear, and proper health care regimens.
